Leveraging the ExpandNet framework and operational partnerships to scale-up brief Cognitive Behavioral Therapy in VA primary care clinics

Evidence-based psychotherapies (EBPs) are underused in health care settings. Aligning implementation of EBPs with the needs of health care leaders (i.e., operational stakeholders) can potentially accelerate their uptake into routine practice. Operational stakeholders (such as hospital leaders, clinical directors, and national program officers) can influence development and oversight of clinical programs as well as policy directives at local, regional, and national levels. Thus, engaging these stakeholders during the implementation and dissemination of EBPs is critical when targeting wider use in health care settings. This article describes how research–operations partnerships were leveraged to increase implementation of an empirically supported psychotherapy – brief Cognitive Behavioral Therapy (brief CBT) – in Veterans Health Administration (VA) primary care settings. The partnered implementation and dissemination efforts were informed by the empirically derived World Health Organization’s ExpandNet framework. A steering committee was formed and included several VA operational stakeholders who helped align the brief CBT program with the implementation needs of VA primary care settings. During the first 18 months of the project, partnerships facilitated rapid implementation of brief CBT at eight VA facilities, including training of 12 providers who saw 120 patients, in addition to expanded program elements to better support sustainability (e.g., train-the-trainer procedures).

The first demonstration of laser action in ruby was made in 1960 by T. H. Maiman of Hughes Research Laboratories, USA. Many laboratories worldwide began the search for lasers using different materials, operating at different wavelengths. In the UK, academia, industry and the central laboratories took up the challenge from the earliest days to develop these systems for a broad range of applications. This historical review looks at the contribution the UK has made to the advancement of the technology, the development of systems and components and their exploitation over the last 60 years.

Computational modeling is an important aspect of the research on nuclear waste materials. In particular, atomistic simulations, when used complementary to experimental efforts, contribute to the scientific basis of safety case for nuclear waste repositories. Here we discuss the state-of-the-art and perspectives of atomistic modeling for nuclear waste management on a few cases of successful synergy of atomistic simulations and experiments. In particular, we discuss here: (1) the potential of atomistic simulations to investigate the uranium oxidation state in mixed-valence uranium oxides and (2) the ability of cementitious barrier materials to retain radionuclides such as 226Ra and 90Sr, and of studtite/metastudtite secondary peroxide phases to incorporate actinides such as Np and Am. The new contribution we make here is the computation of the incorporation of Sr by C-S-H (calcium silicate hydrate) phases.
Shared and specific genetic risk factors for lifetime major depression, depressive symptoms and neuroticism in three population-based twin samples

Background
Vulnerability to depression can be measured in different ways. We here examine how genetic risk factors are inter-related for lifetime major depression (MD), self-report current depressive symptoms and the personality trait Neuroticism.
MethodWe obtained data from three population-based adult twin samples (Virginia n = 4672, Australia #1 n = 3598 and Australia #2 n = 1878) to which we fitted a common factor model where risk for ‘broadly defined depression’ was indexed by (i) lifetime MD assessed at personal interview, (ii) depressive symptoms, and (iii) neuroticism. We examined the proportion of genetic risk for MD deriving from the common factor v. specific to MD in each sample and then analyzed them jointly. Structural equation modeling was conducted in Mx.
ResultsThe best fit models in all samples included additive genetic and unique environmental effects. The proportion of genetic effects unique to lifetime MD and not shared with the broad depression common factor in the three samples were estimated as 77, 61, and 65%, respectively. A cross-sample mega-analysis model fit well and estimated that 65% of the genetic risk for MD was unique.
ConclusionA large proportion of genetic risk factors for lifetime MD was not, in the samples studied, captured by a common factor for broadly defined depression utilizing MD and self-report measures of current depressive symptoms and Neuroticism. The genetic substrate for MD may reflect neurobiological processes underlying the episodic nature of its cognitive, motor and neurovegetative manifestations, which are not well indexed by current depressive symptom and neuroticism.

The patterns of decapod larvae occurrence and abundance were studied from weekly time-series data of 8 years from Stonehaven (north-east Scotland) and 4 years and 8 months from Loch Ewe (north-west Scotland). The annual cycle observed was similar in the two locations and characterized by abundance peaks, the first in spring and another in the summer, extending into autumn. During the coldest months (December to February) decapod larvae were virtually absent in the plankton. Differences in abundance and occurrence of decapod larvae between locations and the influence of temperature, salinity and chlorophyll-a in the patterns observed, were analysed by generalized least-square functions. The results showed significant differences in the abundance of decapod larvae between locations, with higher larval abundances and an earlier appearance in the plankton in Loch Ewe (west coast). In Stonehaven (east coast), from 2003 onwards, a general increasing trend in the abundance of decapod larvae was observed, related to the increasing temperatures recorded at that site. The data demonstrate the high variability of decapod larval abundance on an annual basis and the high importance of temperature and chlorophyll-a to the occurrence and abundance of decapod larvae.

International Ultraviolet Explorer observations of the ROSATWFC-discovered AM Hers RE2107–05, RE1938–46, RE1833–74, RE1149+28, and RE0751+14 reveal UV spectra (1200 – 2000 Å) which are unique when compared with previously studied systems. Specifically, the intensities of individual emission lines due to CIV, CII, N V, HeII, and Si IV, are 2 – 4 times stronger then typically seen. However, the equivalent width ratios of these same emission lines, (CII/CIV, N V/CIV, HeII/CIV, and Si IV/CIV), are similar to those in typical AM Hers. These results indicate that the EUV-discovered AM Hers must have larger UV emitting volumes. Using additional observations of these stars made by EUVE, we speculate on the cause of their larger UV line fluxes.
